The Indiana Complex Care Coordination Collaborative (IC4) is an innovative program built within Indiana University School of Medicine in the Department of Pediatrics' Division of General and Community Pediatrics. The team is an interdisciplinary team of nurses, social workers, doctors and family representatives, working together to improve the care of populations of children with medical complexity (CMC) and adults with intellectual or developmental disabilities (AIDD).
Current projects:
- Coach care coordinators serving children with medical complexity,
- Facilitate improvements in statewide care support to promote coordination of care and access to services which meet the needs of children with special health care needs, and
- Deliver medical education for enhanced primary care skills in caring for persons with disabilities and complex needs.
