The Indiana Complex Care Coordination Collaborative (IC4) is a virtual coaching program and technical assistance center for primary care teams with embedded nurse care coordinators. We work to improve care for targeted populations of patients who are at particular risk for care disparities and unmet needs. IC4's current focus addresses populations of children with medical complexity and adults with intellectual or developmental disabilities.
- Children with medical complexity are defined as children with chronic conditions involving 3 organ systems and requiring at least 3 subspecialists to provide for their care.
- Adults with intellectual and developmental disabilities are defined primarily as adults served by home and community services through the Bureau of Disabilities Services.
IC4 is an innovative program built within Indiana University School of Medicine in the Department of Pediatrics' Division of General and Community Pediatrics. We are an interdisciplinary team of patient and family representatives, nurses, social workers and doctors working together with Indiana Medicaid, Title V Children with Special Health Care Services (CSHCS), The Bureau of Disabilities Services (BDS) and community partners.
