  • The answer may depend on your insurance.
  • Find out if you might be eligible for a new program or plan that has care coordination.
  • Some health systems have programs.

What can you do yourself?

If you don't have a way to get care coordination right now, you can do a little of your own organizing.

If you wish, you can ask someone you trust to help you gather this info.

Bring all of this info when you go to doctor visits.

  • Keep a list of:
    • your medicine
    • your supplies and equipment
    • your doctors and therapists with their phone numbers.
  • Use your calendar for your future visits.

  • Know your medical history:
    • Illnesses
    • Surgeries
    • Major tests
  • Make a list of questions to share with your team.
